Appointments are available Monday through Friday, and some Saturdays.  Your call may be answered by voice mail.  Whether you call for an appointment, or because of an ophthalmic emergency, please leave your name and phone number.  We check our messages frequently and will return your call.  We make every attempt to see patients at their scheduled time.  Emergencies must take priority however, so we ask for your understanding and patience if your appointment is delayed.  Dr. Lindley takes appointments on Wednesdays at our Chicago location, which can be reached at (630)-369-3333.

Fees and Policies

The fees for our services are usual customary for the type of specialty service offered.  Estimates for surgeries are available.  Forms of payment accepted are Cash, Visa, MasterCard, Discover, American Express or Care Credit.  Checks accepted & approved by Telecheck are welcomed.

PAYMENT IN FULL is required for all cases admitted for surgery and or diagnostic procedures.  There is no billing available.

All NEW appointments require a prepaid deposit by credit card. 

As Friday afternoons, and Saturdays are high demand times for appointments with a high cancellation rate, we require a $25 deposit for a revisit appointment to hold your appointment if it is scheduled for either of these two days.  Should you need to cancel or change the appointment, FOR ANY REASON, less than 24 hours in advance of it, this deposit becomes non-refundable.
